Reporting Form: Student Conduct & General Incidents

BEFORE YOU BEGIN: If this is an emergency that involves an imminent risk of harm to self or others, please FIRST contact Auraria Campus Police Dept (ACPD) at 303-556-5000 or 911 from a campus phone. The Auraria Mental Health Line (303-615-9911) may also be used for immediate mental health crises or victim support, by the person in need or for consultation about concerns.

This form is to report Student Conduct concerns or general incidents for which you are not sure where to report. If you are uncertain of what kind of situation you are reporting (e.g. Care, conduct, etc.), please submit the information here and it will be directed as appropriate.

Reports are reviewed during regular business hours and are not monitored after hours, on weekends, or during observed holidays/breaks. While the information will be reviewed and processed as soon as possible, this is NOT designed for emergency response situations.

Thank you in advance for sharing your concerns and/or reporting an incident. For assistance or consultation while completing this report, please contact the Office of Student Conduct and Care at 303-352-3205.
