Incident Report Form


Please provide detailed information regarding the incident you are reporting involving an ISU student. For reports involving ISU staff or faculty members, please contact Human Resources at 812-237-4114.

Only current students, faculty or staff members of the ISU community may file a report via this form. If you are not a current ISU community member, please contact the Office of Student Conduct and Integrity at 812-237-3800 to speak with a staff member. Reports filed through this online Incident Report are immediately received through the system and reviewed the next business day.

If you need immediate assistance, please contact ISU Public Safety at 812-237-5555.

Once the report is received, a university staff member will investigate the incident and determine an appropriate intervention/action plan for the student’s behavior, which may include a meeting with the student and others involved in the incident. Please know that you, and the witnesses you identify in the report, may be contacted to provide further information. ISU does not encourage the filing of anonymous reports. Although thoroughly investigated, clarification or followup will not be possible with an anonymous report.

With regard to reports of alleged Code of Student Conduct violations, university conduct reports are protected by a federal privacy law called the Family Educational Rights and Privacy Act (FERPA), therefore any information regarding the outcome of this report will not be shared with you without the direct written consent of the student(s) involved.

Involved Persons

List the individuals involved and provide information for Name, DOB, and 991. For those who are not from ISU, please list a Drivers License Number (DLN) in the block labeled 991 number. Each Involved Person requires a role:

Respondents:Those who have allegedly violated policy. Complainants:Those who may have been affected by the alleged policy violation. Bystanders:Those who are witnesses. SRP: Conflicts referred to Sycamore Resolution Program

Responding University Officials should not be included in the Involved Persons Section.
