Use this form to submit Code of Conduct and Informational reports. If the incident meets the criteria for more than one type of report, submit the report with the most urgent concerns and indicate all other concerns in your incident description. If unsure, you may submit a report via each appropriate link or utilize this primary reporting form.
Code of Conduct Reports
These reports are received and managed by Student Conduct and should be submitted due to alleged policy violations as described in the Code of Conduct. Please describe, in as much detail as possible, the specific behaviors/alleged violations observed. Be sure to select "Code of Conduct" in the "Nature of this Report" drop-down in the next section.
Informational Reports
These reports are received by Student Affairs and may be routed to departments as necessary. Please describe the situation in as much detail as possible. In the case of roommate disputes, please select "Roommate" for the role of each party. Be sure to select "Informational" in the "Nature of this Report" drop-down in the next section.
