Filing this incident report is only for resolution through Student Conduct and DOES NOT constitute filing an official police report with either the Northern Illinois University Department of Police and Public Safety or the City of DeKalb Police Department. This report may be completed anonymously, by omitting information in any field. Only fields marked with a red asterisk (*) are required. Please know that omitting certain information may impact our ability to fully investigate an alleged incident.
For the "Involved Parties" section of the report, please include as much information as possible. The following definitions are applicable on this form:
- Respondent - A student, student group or student organization who is alleged to have violated the Student Code of Conduct
- Complainant - A person, department, or official entity of the university who submits a complaint or report alleging that a student violated policy
- Victim - An individual who is alleged to have been harmed in an incident
- Witness - incident Witness Any person within the NIU community who has information relevant to an alleged incident or any noncampus community member
- Other Involved - An individual who is not a respondent, complainant, victim or witness but has involvement orrelation to an incident or matter
- Student of Concern - Any student who is displaying behaviors that may impact the student's ability to be successful or safe
- Student Organization - Any student organization
- Unknown - An individual whose involved but not adequately identified in an incident or matter
