Incident Report Form


Any UNC Charlotte student, staff member, or faculty member may submit a report via this form. If you are not a UNC Charlotte community member, please contact Student Accountability & Conflict Resolution at 704-687-0336 or at accountability@uncc.edu . Reports filed are immediately received in our system and are reviewed the next business day. In the event of an emergency or crimes or incidents involving imminent threat of harm, contact UNC Charlotte Police and Public Safety at 704-687-2200.

You will be asked to securely verify your NinerNet credentials before submitting the report. Anonymous referrals may limit the University’s ability to appropriately respond to the referral. Anonymous reports can also be submitted through Police and Public Safety’s Anonymous Crime Reporting form, located at https://police.uncc.edu/webform/anonymous-crime-reporting or through the LiveSafe app. Additional information about LiveSafe can be found at http://emergency.uncc.edu/livesafe

Once the report is received, a University staff member will review the report and determine appropriate next steps. Please know that you and any witnesses you identify in the report may be contacted to provide additional information.

Educational records related to an individual student are protected and shall be maintained pursuant to University Policy 402, Student Education Records (FERPA), including limited exceptions that permit disclosure without a student’s consent. Any information regarding the resolution of this report will not be shared with you without the written consent of the student(s) involved or pursuant to one of the limited exceptions.

Involved Parties

Please list all parties involved (excluding yourself) and provide as much information as possible in the demographic fields. If you do not have access to the demographic information, please continue to submit the report; the information will be researched once the report is received.

For an individual who is not a member of the University community, please list a drivers license number in the field labeled ID Number (if available). If the individual is a student at another college or university, please provide the name of that institution in the field labeled Residence Hall/Local Address.

  • Accused: member of the University community who may have violated University policy
  • Victim: alleged victim or individual who may be impacted by an incident
  • Witness: individual (excluding yourself) who observed the alleged behavior and can provide additional information
  • Involved Party: member of the University community who is involved in an incident and does not fall into the categories of Accused, Victim, or Witness
  • Other: individual who is not a member of the University community

Description

Please provide a detailed description using specific, concise, objective language. Describe, in as much detail as possible, the specific behaviors observed, including words, phrases, and actions, and dates and times. If the involved party used profanity, made threats, or spoke of harming self or others, indicate specific words and phrases used. If the involved party was loud or disruptive, indicate any behaviors associated with the disruption, and describe any injury or damage to individuals or property. Include names of University personnel you contacted and actions taken, if any. For readability, identify individuals in the description by last name.
