This report form is utilized to document incidents of alleged:
- Violation of university or housing policy: Used to inform Housing and Residence Life and/or the Office of the Dean of Students of an alleged violation of policy/behavioral misconduct on or off-campus.
- Crimes: Used to inform University Police and Security Services of alleged crimes ss required by the Jeanne Clery Act.
- Student organization or club misconduct; Reports of Hazing: Used to inform the Office of the Dean of Students of an alleged organization violations of policy/behavioral misconduct on or off-campus.
- General: Informational reports.

- Student of Concern: Submit concern for a student’s health or wellbeing. Used to inform UWSP Care Teams about an individual who maybe struggling emotionally, physically, academically, or exhibiting otherwise concerning behavior.
- Interpersonal Violence: To report allegations of Sexual Assault, Dating or Domestic Violence, Stalking, Sexaul Exploitation, or Sexual Harassment. Full definitions and resources can be found on the UWSP Title IX website.
- Bias / Hate: Used to inform the UWSP Bias Response Team of an incident.
Upon receipt of this report, the appropriate UW-Stevens Point staff member will review the matter and take proper steps to bring resolution to the situation in accordance with our policies and procedures. While you may choose to submit a report anonymously, please understand that while doing so may impact the University’s ability to follow-up if limited information is available.
