Gender Based Discrimination & Sexual Misconduct Incident Report


Life University is committed to creating an environment that is free from sex discrimination (including pregnancy and parenting status), gender-based harassment, sexual harassment, sexual violence, domestic/dating violence, stalking, and retaliation. This form should be used to report incidents involving alleged violations of the university's Sexual Misconduct Policy.

Incident reports will be processed as soon as possible and a member of the Title IX Office will contact you within one to two business days of the report submission.
