Utilize the form below to submit reports of concerning non-academic behavior that is NOT related to cheating or plagiarism (e.g. alcohol, drugs, vandalism, disorderly conduct, worrisome and/or threatening behaviors).

Involved Parties

Please list the individuals involved, including non-students. Please do not include yourself as an involved party.

For students, please complete all the designated fields.

For non-students, provide as much information as possible.

If you do not have access to the demographic information, continue to submit the report. The information will be researched once the Incident Report is received.

An individual/organization listed as Respondent is the party who may have violated policy. Ones listed as a Victim are the individual/organization to whom the misconduct was directed; ones listed as a Witness are an observer to the incident. An Individual listed as a Student of Concern is a student who was not involved in an alleged policy violation(s), but may need to be connected to additional University resources.

Description

Please provide a detailed description of the incident/concern using specific concise, objective language. If you have listed an individual as an involved person, he or she should appear at least once in the narrative.

For matters involving Conduct violations, please provide as much detail as possible in describing the behaviors and activities that occurred. The use of direct quotes, including profanity or abusive language, is encouraged. Avoid using pronouns when multiple individuals are involved. These reports are submitted to the Dean of Student Affairs for resolution.

For FYI reports, please include as much description as possible regarding the circumstances that resulted in the report.
