This report is designed for instructors to report cases of academic dishonesty. Any students reporting academic dishonesty should do so here. Students may also email integrity@purdue.edu to report a matter to us. Any questions about this process should be referred to the Office of Student Rights and Responsibilities (OSRR) at 765.494.1250 or osrr@purdue.edu.

Instructors are strongly encouraged to confront the situation within your class and meet with the student to discuss the allegation of suspected misconduct. For additional information on meeting with a student regarding an issue, please see the Faculty Guide for Responding to Academic Dishonesty.
Once you've made a determination as to what action will occur within the class, please use this web submission form to report the matter to the Office of Student Rights and Responsibilities for additional follow-up and record keeping within the institution.
Please upload any supporting materials and/or information that you have regarding the situation electronically, under the “Supporting Documentation” section. This includes a copy of your course syllabus and copies of the exam, paper or other class document in question, plus any other materials that support the allegation.
Once the report is received, the OSRR will make contact with the student to have them come in and discuss the matter. We will determine what additional action, if any, will be taken with the student under the Code of Student Conduct. Examples of additional action that may be taken include placing the student on a disciplinary probation status with the university for a period of time, issuing an administrative warning, having the student complete a paper on the Purdue University Honor Code, having the student complete an ethical decision making seminar, or other appropriate educational sanctions. In cases of egregious academic misconduct, or in cases where the student may have a pattern of misconduct within the university, the student may be separated from the university because of their behavior.
If requested, once the case has been adjudicated by the OSRR, we will follow up with the reporting instructor to inform them of the outcome and any actions that were taken with the student pertaining to the incident.
