Student Conduct Report Form: Academic Misconduct


This form is used to document and report Academic Misconduct as defined in the the Student Conduct Code, Article I, o. Other forms of student misconduct should be reported on the general Student Conduct Report Form.

Once you submit the form, it will be sent to the Office of the Vice President for Student Affairs, whose staff will send a letter to the accused student informing him/her of the charges, classroom sanctions, and due process rights as provided by the Student Conduct Code.

Please note:
• If the student accepts the charge of academic misconduct and the classroom sanction(s) as assigned, no further action will be needed on your part.
• If the student contests the charge and/or classroom sanction(s), a Student Conduct Board Hearing (SCB) will be held to determine responsibility, and you will be asked to serve as a witness.
• Beyond the classroom sanction(s) you assign below, further sanctions may be imposed by the Student Conduct Officer and/or the SCB.
• Faculty are encouraged to consult with the department chair/director prior to submitting this form.

Supporting Documentation

Supporting documents may include syllabus, turnitin.com reports, images, audio and video files, etc. These are often critical to being able to demonstrate the academic misconduct should this case to a Student Conduct Board for resolution. Video files larger than 5GB should be shared with Dan Rodkin via SF's OneDrive system. Evidence you are unable to submit electronically should be brought to the Conduct Officer in Building R, room 211. 5GB maximum total size.
