If you have information from a person who wants to remain anonymous, please schedule a consultation with the Office of Community Standards to determine whether the matter may continue forward. Do not enter that individual's name on this form or include reference to information from that individual in the Letter of Concern.
All information you provide is shared with the Responding Student, and eventually given to a panel if the concern proceeds to a hearing. Please keep in mind that this process is otherwise confidential.
