Academic Integrity Violation Reporting Form


According to the bylaws adopted by the Faculty of Arts, Sciences, and Engineering, including the School of Museum of Fine Arts, in the spring of 2006, faculty members are required to report all suspicions of academic misconduct to the Dean of Students Office. Please use this form to report all suspected academic misconduct. Please also feel free to use this form to report other academic or classroom issues to the Office of Community Standards and the Dean of Students Office.
