Referral Form


Please provide a detailed description of the concern using specific, concise, and objective language (who, what, where, when, why, how). Include all relevant information about why you believe that a violation of the Code of Academic Integrity occurred. In addition, please attach all relevant supporting documentation (including copies of the coursework in question) to the form before submitting.

Please note: The information you submit will be made available to the student(s) referred as part of the adjudication process. Students will receive an exact copy of the report and documents you submit. Any information from an anonymous source will not be considered relevant and cannot be considered in the determination of the outcome of the case. 


In our experience, it is best for the student(s) to hear about the referral directly from you before they hear about it from us. A quick email to the student is recommended.

Please include any recommended outcomes (consequences). These recommendations will be taken into consideration, however, the final decision is made by the Coordinator of Academic Integrity or the Academic Integrity Council (if the case goes to a formal Council Meeting).

Please do not submit a final grade for the referred student(s) until the conclusion of the case. Referring faculty will be copied on the decision letter sent via email to the student.
