Appeal a Conduct Officer Decision


Please use this form to appeal the decision of a conduct officer after a conduct officer hearing.

As you have chosen to appeal, the burden of proof shifts to you.

The Appeals Board will review your appeal, along with the information in your conduct file, and any new evidence you submit that wasn't available at the time of your initial hearing. If you have additional information you wish to submit to the Appeals Board, you must do so at the same time you submit the appeal.

Please note that the appeals board will not hold a live hearing, but will meet independently to review your appeal.
