If you are concerned about a person and/or their behavior, complete the form below. Anyone can share a concern.
Please provide detailed information about your concern. When we receive your form, the receiving office will review the information, develop an action plan, coordinate responses, and connect students to campus support or community resources.*
Please select the nature of the report according to the following criteria:
- Student concern: Share student well-being and/or safety concerns. Students may be struggling emotionally, psychologically, and/or physically. Reports go to the CARE Team*
- Misconduct by student: Report possible student violations of the BC Student Conduct Code. Reports go to Student Conduct
- Sexual harassment: Report incidents of sexual harassment, stalking, domestic/dating violence, and sexual misconduct. Reports go to Title IX
- Employee concern: Report incidents or share concerns about faculty or staff members. Reports go to Human Resources
- Bias or hate: Use the online Bias/Hate Incident Report to report incidents
- Academic integrity: Use the online Academic Dishonesty Report for cheating, plagiarism, or other academic integrity concerns
* A CARE Team member can contact a student to check in, to provide support/resources, and/or to make a Counseling Center referral based on a concern. However, a BC counselor cannot typically reach out to a student based on a CARE Form due to their state licensure requirements and professional ethical standards.
