Choose General Incident Report for concerns or actions that are experienced or observed and present a danger to an individual or the Central Methodist University community.

 

Choose Housing and Residence Life Report for concerns or actions that are experienced or observed in a living environment and present a danger to an individual or the residence hall community or violate code of conduct or house rules - All Residential Student Staff should use this category for all reports.

 

Choose Public Safety Report for reports being submitted by the Public Safety staff members.

 

Choose Concerning or Threatening Behavior Report for concerns or actions that are experienced or observed and pose a high, but not immediate, risk for harm or danger to self or others

 

Choose Human Resources Report for incidents and accidents arising out of and in the course of employment, regardless of the presence of an injury or illness.

 

Choose Liability Report for incidents and accidents involving university-owned property

 

Choose FYI Report for concerns or actions that are experienced or observed and are worrisome but do not present an immediate danger to self or others.
