Incident Reporting Form


The form below will assist individuals to report student concerns both on- and off-campus to Student Affairs and/or the Center for Honors Enrichment and Community Standards (CHECS). If the concern involves a possible violation of the Code of Conduct, CHECS will review. If the concern involves a mental health or FYI related report, the appropriate staff within Student Affairs will be notified to review. Anonymous reports will be considered, however, reports that do not include identifying information may limit the university's response and ability to follow-up or corroborate information.

Do not use this form to report violations of the Discrimination, Harassment and Sexual Misconduct Policy. All reports of discrimination, harassment, or sexual misconduct should be directed to the Title IX and EO Community Reporting Form or the Sexual Misconduct Responsible Employee Reporting Form.

Please do not use this form in place of contacting CNUPD (757-594-7777) in an emergency. Call CNUPD and complete this form at the conclusion of your role in that incident.
