Behaviors of Concern Report
Please use this form to report violations concerning Student Code of Conduct, Sexual Harassment, Discrimination (Title IX), and general student behavior concerns. In the form below there will be a drop-down menu for you to select your specific concern, and that will determine how your form is routed.

Reports of Sexual Harassment and/or Discrimination will be received by the Title IX campus coordinator. Reports of general behavior concerns are received by the Senior Director of Student Success & Development and reviewed by the Behavioral Intervention Team (BIT). For some reports, the BIT will assist in coordinating resources and implementing a centralized response with the goal of providing assistance to the student(s) and affected campus community members. The BIT conducts risk management and implements collaborative, community-centered interventions with the aim of maintaining a safe and productive learning environment.
Reports are reviewed during normal business hours and are not monitored after hours, on weekends, or during official college holidays.
Please provide as much information as possible. This maximizes the opportunity to offer the most effective, appropriate interventions. Referrals made using this form do not violate student privacy rights.
