Frederick Community College (FCC) students must be familiar with and follow the Code of Student Conduct. FCC community members may report violations of the Code of Student Conduct or submit reports for behaviors of concern.  Please provide detailed information regarding the incident you are reporting. Reports may be anonymous. However, anonymous reports may limit the College’s ability to respond to the incident. Once the report is received by the Office of the Associate Vice President for Student Affairs, it will be reviewed to determine the next steps, which may or may not include contacting the student, you, and any witness you have identified. Frederick Community College student records are protected by a federal privacy law called the Family Educational Rights and Privacy Act (FERPA).

Academic Integrity Report Form

Select this form to report incidents of cheating, plagiarism, or facilitating dishonest academic activities.  Academic Affairs will conduct report follow-ups.

 

Athletics Incident Report Form

Student-athletes can use this form to report concerns regarding safety and successful participation in athletic activities based on the Student-Athlete Concerns about Athletic Programs and Activities Policy and Procedures.

 

Code of Conduct Incident Report Form

Select this form to report prohibited behaviors as noted in the Code of Student Conduct, including, but not limited to, college technological equipment & resources, alcohol, tobacco & other drug use, disruptive behavior, fighting, and threatening behavior.  This form should not be used for academic or Title IX violations.

 

 Behaviors of Concern Incident Report Form

Select this form to report incidents that note behaviors of students that can include but not be limited to displays of heightened emotional distress, disturbed writing, suicidal remarks, mood changes, or concerns/wellness of students. These reports may be referred to the Office of Counseling & Wellness Services or the Behavioral Evaluation & Response Team (BERT).

 

Sexual Misconduct/Pregnant & Parenting Accommodations (Title IX) Incident Report Form

Select this form to report incidents related to the Title IX: Sexual Harassment Policy violation. This includes sexual assault, domestic violence, dating violence, and stalking.  Additionally, this form should be submitted by students seeking support under the Title IX: Pregnant & Parenting Accommodations Policy.   Title IX reports involving students will be reviewed by Dr. Benita Rashaw.  Title IX reports involving employees will be reviewed by Daniel West.

 

Death of a Student Incident Report Form

Select this form to report incidents regarding the death of a current student.
