If you want to refer a student for ADA or Title IX:Pregnancy and Parenting concerns, please complete the following form. This form is submitted directly to Student Access, which allows Student Access staff to follow up directly with the student.
ADA or Title IX: Preganacy and Parenting concerns such as Injury or medical issue, surgery or hospitalization, extended medical illness, disordered eating/ sleeping, substance abuse or concerns related to alcohol or drug use
Report Types:
There are 4 different types of BIT report options you can select for submission:
- Community Standards Report -- Submitting this report alerts the appropriate individuals at HACC regarding potential violations of the Student Code of Conduct.
Description: The Office of Community Standards wants the HACC community to be safe and ensure any and all violations of the Student Code of Conduct are reported.
Upon receipt of this report, the appropriate HACC administrator(s) will review the matter and take proper steps to address the concern in accordance with HACC policies and procedures. HACC respects the sensitivity of the information that may be included in this report and will make all reasonable efforts to protect the privacy of those involved, in accordance with applicable state and federal law, while balancing the need to gather information to address the incident in order to protect the HACC community.
This form may not be reviewed outside of normal business hours. - Academic Dishonesty Report -- Submitting this report alerts the appropriate individuals at HACC regarding potential violations of Academic Dishonesty
Description: Academic, research, and scholarly integrity is of the upmost importance to HACC. Employees and students share responsibility in preserving the integrity of the academic experience at HACC. Academic dishonesty damages the educational experience and ultimately hurts many parties, including employees, other students, and the value of HACC credits and degrees. Students and employees are encouraged to understand the expectations outlined by the Student Code of Conduct and professional standards of academic colleges and programs, to report suspected incidents of academic misconduct, and to hold each other to a high standard when it comes to the integrity of academic work.

- Student of Concern Referral Report -- Submitting this report alerts the appropriate individuals at HACC regarding Need for student support and resources.
Description: The BIT Team is a central network of HACC’s campus community focused on caring, prevention, and early intervention for students experiencing serious distress or engaging in harmful or disruptive behaviors. You should submit a Student of Concern Referral Report if you encounter a student who may be experiencing any of these:
- ACADEMIC CONCERNS: Excessive class absence concerns, noticeable changes in academic performance, failure to respond to faculty/ staff outreach, student wishes to withdraw from university
- SOCIAL CONCERNS: Homesickness or adjustment concerns, feelings of sadness, loneliness, or isolation, roommate issues, feeling of not belonging/ lacking campus connection, family or support system issues, relationship issues
- BEHAVIORAL/ EMOTIONAL CONCERNS: Expressions of severe anxiety or anxiousness, written or verbal statements of distress or despair, suicidal ideation or attempt, self-harming threats or behavior, inappropriate emotional outbursts, unprovoked anger/ aggression, disturbed writing, threat of harm to others
- RESOURCE CONCERNS: Food insecurities, housing insecurities, financial insecurities, legal concerns, school supplies or textbook concerns, student requesting resources, other (please specify in narrative below)
- ABSENCE CONCERNS: Death of family member or loved one, student impacted by traumatic event or legal concerns, unable to locate student, welfare check occurred
If you sense something that does not feel right, say something to a BIT Team member or do something by submitting a report to help a student. - Discrimination, Harassment, Bias-Motivated Acts/Behavior, Sexual Misconduct and Title IX Report -- Submitting this report alerts the appropriate individuals at HACC regarding potential violations of sexual misconduct including sexual assault, sexual harassment/discrimination, dating/domestic violence, stalking, and sexual exploitation
Description: This form should be used to report incidents of alleged discrimination, harassment, including Bias Incidents, and all forms of sexual misconduct including sexual assault, sexual harassment/discrimination, dating/domestic violence, stalking, and sexual exploitation. Students, employees, and non-community members are encouraged to use this form if they believe that they have been impacted by, were a witness to, or have information of a potential violation of HACC’s Discrimination, Discriminatory Harassment and Bias-Motivated Acts and Behavior Policy.
Employees who are required reporters must use this form to report all known details of potential policy violations if an incident was disclosed to them by a student. All required reporters must provide their own name when submitting a report.
