Student Conduct Reporting Form


The form below is intended for the sole use to report potential violations of the Student Code of Conduct to officials at Westchester Community College. This form is to be used when a community member has received information regarding an incident that they, in good faith, believe demonstrates a violation of the WCC Student Code of Conduct. Please fill in as many of the fields below. Please be as complete as possible to provide the College with the best opportunity to respond to this incident. In all cases the person reporting the incident to the College may choose to withhold their identity and report anonymously; however this may impact the college's ability to respond to the report.

This report is reviewed during normal business hours and should not be used if you need emergency assistance.
