Saint Louis University utilizes a variety of reporting options to ensure that a report is directed to the the most appropriate office for a response. Please review the following list of options to determine which reporting option fits your needs.

Please provide detailed information that you are reporting involving a Saint Louis University (SLU) student. For all emergencies contact DPS at 314-977-3000. Please do not use this form to submit information regarding Faculty or Staff. For Faculty and Staff concerns, please report your concerns to the SLU Office of Compliance and Ethics .

Student records, including incident reports, are protected by FERPA (Family Educational Rights and Privacy Act), therefore any information regarding the outcome of this report will not be shared with you without the direct written consent of the student(s) involved. Any exception to this policy can be found in the Student Handbook .

This report will be received by The office of Student Responsibility and Community Standards for review. This form should be utilized to report an issue/concern/or incident that violates the University's Community Standards outlined in the Student Handbook.
This form should be used by anyone who have been the targets of, or who witness, a hate or bias based on (sex, race, color, religion, national origin, ancestry, disability, age, sexual orientation, marital status, military status, gender expression/identity, genetic information, pregnancy, or any other protected classes) incident by a student.
Use this form to submit concerns about a student’s health, well-being, and/or success. Concerns regarding mental health, hospitalization/illness, loss of a loved one, basic needs, academic engagement, and other issues impacting a student’s overall well-being may be reported here.

Immediate threats of harm to self or others is an emergency and should be directed to the Department of Public Safety (DPS) at 314-977-3000. The Behavioral Concerns Policies and Procedures can be found in Section 4.0 the Student Handbook.

Reports can be made anonymously via this form. If you chose to submit this form anonymously, please understand our ability to take action may be limited given the availability of the information provided. The Dean of Students Office is staffed during business hours (Monday-Friday from 8:30am - 5:00pm, excluding university holidays).

This report will be reviewed by the Title IX Coordinator, or their designee. This form should be utilized to report any allegations or incidents involving Sexual Harassment, which are defined as Prohibited Conduct and include: sexual harassment, sexual assault, dating violence, domestic violence, sexual or gender-based stalking against University community members of any gender, gender identity, gender expression, and sexual orientation.
This form should be utilized to report an issue/concern/or incident that does not meet any other criteria in the list. Fire alarms, pet violations, mediation requests, and facility related matters, etc. should be reported using this form.
