Please use this form to report any behaviors of concern involving University of Cincinnati students, whether these behaviors occur inside or outside of the classroom setting. Reportable behaviors may include Student Code of Conduct Violations, a student who you feel may be struggling physically or psychologically, or any other behaviors of concern that you would like to see addressed or would like to discuss. When completing this form, please include your name, title, and contact information. Please note that anonymously submitted forms may limit our effectiveness in dealing with concerning behavior.
Once a report has been submitted, the student(s) will be contacted by a staff member to address the alleged misconduct or concern. Please know that you and any witnesses you identify may be contacted to provide additional information. Please note that student conduct records are protected by the Family Educational Rights and Privacy Act (FERPA) of 1974.

When selecting the nature of the report below, please use the following definitions:
Hazing for concerns of possible hazing within student organizations, identifiable student groups or teams.
Student Code of Conduct for alleged misconduct that occurs on or off campus involving UC students or student organizations and may violate the Student Code of Conduct.
Student of Concern Committee CARE Team, or incidents that you would like to report about a UC student who you feel may be struggling physically or psychologically.
Student Health Insurance Waiver Appeal, Student appealing their health insurance.
I don't know for incidents that you are unsure of where they fit. A staff will direct the report to the appropriate office.
