Student Conduct Incident Reporting Form


Please use this form to report alleged violations of the Student Code of Conduct. Submitting this form initiates the Student Conduct process. Supporting documentation (screenshots, recordings, etc.) should also be included. These items can be attached to at the end of this form.

PLEASE NOTE: SCCS oversees the student conduct process for UTK. SCCS cannot discipline faculty, staff, or non-students.
