Academic Integrity Incident Report Form


This form should only be completed by current Appalachian State University faculty who want to report an alleged violation of the Academic Integrity Code. If you are not a faculty member, please contact the Office of Student Conduct at 828-262-2704.

Reports filed through this online Incident Report are immediately received through our system and reviewed the next business day. In the event you need this report to be reviewed immediately, please contact the Office at 828-262-2704.

The Office of Student Conduct does not accept anonymous reports about students. Once the report is received, a university staff member of the Office of Student Conduct will verify that you may resolve the alleged violation with the student via the faculty/student resolution process, or if the case needs to be referred to the Academic Integrity Board.
