This form is for ACC students, employees, and community members for concerns related to mental and physical health, behavior concerns, or complaints. Please provide detailed information regarding your concern below. Once this referral is received, a member of the ACC Care Team will review the information and take appropriate action, as necessary. If you enter your information, a team member will follow up with you regarding your referral. If you prefer, you can call the Dean of Students Office at 303.797.5730 or Room M2720 Littleton Campus, to refer your concern.

Faculty and Instructors, Academic Alerts or Concerns related to the progress of a student in class or observed behaviors that may lead to academic difficulty in a particular class should be reported through Navigate: Staff, not this form. For example, excessive absences/tardiness, not logging into class, not responding to emails/texts, not completing homework, lack of class participation, poor time management, or struggling with keeping pace in class.
