Sexual Misconduct or Title IX Incident Report


This form provides preliminary information to the BYU-Hawaii Title IX Coordinator's Office to assist in resolving violations of BYU-Hawaii’s Equal Opportunity and Non-Discrimination Policy. Please complete the form to the best of your knowledge.

Once you submit this report you may be contacted by someone from the Title IX Deputy Coordinator's office if additional information is needed. Regardless of the nature of your report, the university will ensure that your information is forwarded to the office with responsibility for investigating and addressing complaints, reports, or concerns as appropriate, based on the information you have provided. The information you provide will be kept confidential to the extent permitted by law.

BEFORE YOU BEGIN: If this is an emergency that involves an imminent risk of harm to self or others, please call Local Police by dialing 911, or BYU-H Campus Security at 808-675-3911 prior to filling out this referral form. Referrals are reviewed during normal business hours and are not monitored after hours, on weekends, or during official University holidays. While referrals from this form are reviewed by a variety of campus partners working to assist students and employees, it is NOT designed for emergency response situations.
