Student Conduct Incident Report Form

The Dean of Students office at CSU Channel Islands is charged with upholding the Student Conduct Code, which identifies behavioral expectations for all students on and off campus. If you have knowledge of a CI student that may have violated the Student Conduct Code, you are encouraged to notify our office of the incident. Please use this form to report any behaviors that violate the Student Conduct Code. Once you submit the form it will be reviewed and you may be contacted for additional information.

If you are concerned about the health, safety, and/or well-being of a student and would like the student to be connected to available campus resources please submit a report here: Referral to University CARE Team

Involved Parties

For students, please complete all the designated fields.

For non-students, please list a Drivers License number in the block labeled ID Number if available. If the non-student attends another institution, please list the name of the institution in the block labeled Hall/Room or Address.

If you do not have access to the demographic information, continue to submit the report. The information will be researched once the Incident Report is received.

An individual/organization listed as Accused/Respondent is the party who may have violated policy. Ones listed as a Victim/Complainant are the individual/organization to whom the misconduct was directed; ones listed as a Witness are an observer to the incident.
