Reporting an Incident
Filing this report constitutes official notice to Cal Poly Pomona. The University respects the rights of Complainants to choose the resolution path that is best for them. The submission of this form, on its own, does not constitute a Formal Complaint. Cal Poly Pomona strongly supports and encourages prompt reporting of sexual harassment, sexual misconduct, dating/domestic violence, sexual exploitation, stalking, and DHR incidents. Reporting provides resources to Complainants and contributes to keeping the campus safe.
After submitting this report, you or the person you are reporting about will be contacted by the Office of Equity and Compliance (OEC) staff to schedule an initial meeting to discuss rights and options for supportive measures and filing a Formal Complaint. Title IX
Instances of sexual misconduct/sexual harassment may violate both the CSU Nondiscrimination Policy and the law. If you have experienced or have received a report of sexual misconduct/sexual harassment there are several options a Complainant may pursue at any time.
Discrimination, Harassment, and Retaliation (DHR)
Cal Poly Pomona committed to maintaning a campus environment free from discrimination, harassment, and retaliation for students, faculty, staff, and third parties.
Discrimination or harassment based on protected status is prohibited under University policy, as well as federal and state law.
Retaliation for engaging in a protected acitivity is prohibited under the CSU Nondiscrimination Policy. All reports of retaliation will be taken seriously and reviewed with the utmost diligence.
Confidentiality
Reports submitted to the Office of Equity and Compliance will be kept confidential to the extent possible and will be shared exclusively with University officials on a "need to know" basis.

Criminal Report
Filing this report DOES NOT constitute filing an official police report with the Cal Poly Pomona University Police Department. To file an official police report, please contact Cal Poly Pomona University Police at (909) 869-3070 or in-person at 109 Cypress Ln (Bldg 109, adjacent to the parking structure).
Responsible Employee
All faculty members, staff members, and administrators, with the exception of those working in a confidential capacity (e.g., counselors, rape crisis advocates, etc.) who know or have reason to know of incidents that may violate the Nondiscrimination Policy, are required to disclose all information available, including the name of the parties involved, even where the person has requested anonymity. Please refer to the Nondiscrimination Policy for more information on Responsible Employees.
If an online report form is submitted outside of normal business hours, OEC will coordinate a response within 1-2 business days upon our return.
