Sexual Discrimination, Harassment, and Misconduct (Title IX)


This form provides preliminary information to the Office of Student Conduct and Office of Organizational Culture & Engagement to assist in investigating reports on alleged violations of Chattanooga State's Policies on Sexual Discrimination, Harassment, and Misconduct. Please complete the form to the best of your knowledge.

For purposes of reporting, sexual violence means contact of a sexual nature perpetrated against a person's will or where a person is incapable of giving consent due to drugs, alcohol, or disability. Sexual violence includes sexual assault (rape), sexual misconduct, sexual coercion, gender-based harassment, intimate partner violence, and stalking.


This report form can be utilized by students wishing to file a report on their own behalf or by anyone wishing to file a report on behalf of the College.
