Orange Coast College Incident Reporting Form


Please use this form to submit allegations related to any of the following:
-Student Conduct Violation
-Academic Dishonesty
-Behavioral Assessment Team (BIT) Referral (Concerning behaviors)
-Sexual Misconduct *

* Orange Coast College will provide a prompt and equitable response to reports of sexual misconduct, including (1) inform the impacted party of the availability of supportive measures, (2) consider the impacted party's wishes with respect to supportive measures, and (3) explain the process for filing a formal complaint. Please note this form does not constitute a formal complaint, pursuant to District policy AP 5910 Sexual Misconduct; for more information please refer to Section 9.6.
