This form is designed to provide an online method to report sex discrimination, including sexual harassment, sexual assault, dating violence, domestic violence, and stalking. Any person may submit a report at any time.
Orange Coast College will provide a prompt and equitable response, including:
- Inform the impacted party of the availability of supportive measures,
- Consider the impacted party’s wishes with respect to supportive measures, and
- Explain the process for filing a formal complaint.
Supportive measures are individualized services to help restore your access to Orange Coast College’s programs and activities. Examples include academic assistance, contact limitations, employment assistance, referral to counseling or other healthcare services, and safety planning, including on-campus Public Safety escorts. To access supportive measures, please contact Shannon Quihuiz, Associate Dean, Student Relations & Title IX at squihuiz@occ.cccd.edu.
Please note this form does not constitute a formal complaint pursuant to District policy AP 5910 Sexual Misconduct; for more information please refer to Section 9.6.
