This form is used by faculty to report alleged academic dishonesty violations.

The Dean of Students Office encourages faculty to use this online form to submit all relevant materials, including:

A copy of the course syllabus;
All materials related to the alleged violation;
Any relevant communications between the faculty and the student

All information shared and submitted on this form will be reviewed with the student in question.
