Academic Dishonesty Reporting Form


This form is used by faculty to report alleged academic dishonesty violations. 

The Dean of Students Office encourages faculty to use this online form to submit all relevant materials, including:

  • A copy of the course syllabus;
  • All materials related to the alleged violation;
  • Any relevant communications between the faculty and the student

All information shared and submitted on this form will be reviewed with the student in question.
