Academic Integrity Report


Please use this online form to report your findings for an academic integrity violation to the Office of Student Rights and Responsibilities. Prior to submitting this form, you must have addressed the academic integrity violation as outlined in the East Carolina University Faculty Manual found at https://www.ecu.edu/cs-acad/fsonline/customcf/currentfacultymanual/part6section2.pdf.

This form must be submitted to OSRR within 10 calendar days of the initial meeting with student.

 

If the student waives their right to a departmental meeting, the Academic Integrity Waiver must be attached to this report. If the student does not waive their right and a departmental meeting occurred, the Chair or designee MUST be the one who fills out the reporting form. The report should indicate the date the departmental meeting occurred and who made the decision regarding the case. We cannot process the case without the waiver or a resolution from the Chair or designee.
