Action: A suspicion of an academic dishonesty incident can be reported by either students or instructors. In response to a suspicion of a violation of academic integrity standards please fill out the below information. For an instructor initiating the report, the following options are available:
(a) Dismiss the matter: If the instructor concludes that there is no violation of these rules, the matter is over.
(b) Resolve Internally: If the instructor believes that the student committed a Class I violation of academic rules, (see below for class definitions), the instructor may take one or more of the following actions without entering an official violation.
- Instruct the student on academic integrity standards and explain how the student failed to comply with those standards;
- Allow the student to modify or redo the assignment; and/or,
- Provide the student with an educational opportunity to reiterate academic integrity (such as an assignment, research, course or tutorial on academic integrity).
NOTE: Do not submit this form if there are not suggested grade sanctions. Instructor of record should maintain documentation of internal resolutions without grade sanctions. If an instructor intends to impose any sanction that will affect the student's course grade, he/she must initiate the Summary or Board review process. Internal resolution may not be used in such cases. * Please also note that preparing work for intented use by another is also a violation of academic integrity. If you believe it more likely than not that a student assited another student in the commission of a violation, you are strongly encouraged to submit an incident report.
(c) Initiate the formal Academic Integrity Process: Summary or Board If the instructor believes that the student violated academic integrity standards and internal resolution is not appropriate.
If the Academic Summary Process is selected as the instructor resolution method, then the student alleged for a policy violation will be sent a summary notice that presents them with the following choices:
- CONCUR: A student can decide to accept responsibility and the sanctions proposed by the instructor.
- CONFERENCE: If a student disagrees with what is being alleged, and has not discussed the situation with the course instructor, the student will be presented with the option to request a faculty conference with the instructor in order to discuss the alleged violation and/or proposed sanction. If a conference is agreed to, the student and instructor will attempt to reach a resolution within seven (7) calendar days from initial notification of the meeting request.
- AI REVIEW BOARD: If a resolution cannot be met, the student or the instructor can request that the case be referred to the Academic Integrity Board for review. The AIB will review the student’s written statement and evidence submitted by the instructor or student to determine if the student is or is not responsible for an academic integrity violation.
If Academic Integrity Board review is selected as the instructor resolution method, the case will be referred directly to the Academic Integrity Board for review, and a notice of the pending review will be sent to the student. Copies of review notifications will also be sent to the reporting instructor.
NOTE: Parties suspected or accused of acts of academic dishonesty receive a copy of the submitted incident report detailing the allegation(s). Please submit individual incident reports for individual parties unless working together or collusion is suspected/alleged.
