Discrimination and Harassment Report Form; Including Sexual Misconduct and Sexual Harassment

This form is designed to provide the Eckerd College community, which includes, but is not limited to students, student organizations, faculty, administrators, staff, guests, visitors, invitees, and campers with an on-line method to report specific information related to an alleged incident(s) of sexual misconduct, harassment, discrimination, which can encompass sexual violence, stalking or intimate partner violence. This form may be used to file a report for yourself or on the behalf of another person.

The College will use the information provided to begin a preliminary inquiry and/or investigation, which may include contacting the reporting party, complainant, responding party and/or any potential witnesses.

A note about anonymous reports:

Please note that the College can address submissions most completely when we are able to communicate with people reporting. The benefits of direct reporting include a greater ability for Eckerd to understand the situation, provide resources, support, and options to those reporting, and to address the behavior fully and accurately.

We do accept anonymous reports. Those wishing to report anonymously may submit this form without identifying information. If you choose to submit a report anonymously, please note that Eckerd will strive to fully respond, but anonymous reporting significantly limits our ability to do so.

If you submit this form with your name or contact information, a staff member may contact you to verify receipt of the report, to ask any clarifying questions they may have, and to offer you relevant resources.

The Title IX Coordinator has the primary responsibility for coordinating efforts related to investigation, resolution, and implementation of corrective measures and monitoring to stop, remediate, and prevent discrimination, harassment, and retaliation prohibited under this policy. The Title IX Coordinator acts with independence and authority free from bias and conflicts of interest. The Title IX Coordinator oversees all resolutions under this policy and acts to assure that all Eckerd College representatives act with objectivity and impartiality and are assessed with respect to conflicts of interest and/or potential bias.
