Embry-Riddle has provided this form to allow anyone internal and external of the University to report students who are of a concern to themselves and/or to the community.
For LIFE THREATENING emergencies, please call Campus Safety at (386) 226-7233 and then 911.
If you have an immediate concern about a student’s health or safety, please call Campus Safety at (386) 226-7233. A Campus Safety Officer and/or appropriate staff will quickly respond to the situation.
When you feel a student is of concern in any area of their academics, health, mental well-being, and life and/or may need referral to resources, please submit the Student of Concern Form. Provide detailed information about the circumstances you are reporting to allow University staff to determine the best course of action to support the student.
If you are unsure of the appropriate way to help a student, call the Dean of Students Office, (386) 226-6326, during normal operating hours and Campus Safety, (386) 226-7233, after hours.
University staff will respond and follow up with students of concern, providing referrals or general support. Again, life threatening emergencies require a call to Campus Safety and 911.
Please note: Response, actions and outcomes are protected by multiple privacy regulations and the reporting person may not be privy to the steps or outcomes of this report.
