Admissions Clearance Reporting Form


At this time, your application has been referred to the Admissions Clearance Committee because you indicated a positive response to the question regarding past academic/behavioral misconduct and/or criminal history on your application to Florida International University. Please complete this form and submit the required documents in order for a recommendation to be issued.

Once all the necessary information is received, the application is reviewed and a recommendation made. In some instances it is determined that the information submitted be reviewed by the Admissions Clearance Committee, which meets on a monthly basis as needed.
