Housing Incident Report


If this is an emergency situation, contact the Florida State University Police Department at 850-644-1234. This form should not be the primary means of reporting an emergency situation in progress.

Staff are reminded to inform Head Staff On-Call for any incident where FSUPD is contacted, regardless if FSUPD takes any action or provides a police report number.

Involved Parties

Please list the individuals involved, including witnesses, and for each of them, please include as many of the listed fields as you can provide. If you are documenting a building or an incident that has no known students involved, please use the name of the building followed by "Hall." For the ID Number, please used the name of the hall. For example, an incident that occurs in Broward Hall should have Broward Hall listed as the involved person's name and "Broward" as the ID Number.

Do NOT list yourself or other staff, or police/emergency officials in this section (but they should be identified in the narrative below).
