Early Academic Alert


An Early Academic Alert is a tool used to intervene in the event that a student is showing signs that they may not be successful during their course of study. Instructional staff is encouraged to make initial contact with the student when situations arise. When the instructor finds such contacts ineffective, referral to Counseling & Advising Services is appropriate.

Student(s) of Concern

List the Name and Student ID number of the students that you would like to list in this Early Academic Alert. You may list up to 99 students on one Early Academic Alert Form; however, the reason for the Alert must be the same for all students on the Alert Form. For example, if there are 4 students in one class that you wish to list on an Alert Form due to a lack of attendance, then you may list them on the same form. However, if there is one student of concern due to missing assignments, two due to missing assignments and another due to behaviors, three separate Alert forms would need to be filled out.
