Students who are alleged to have violated the University Housing Prohibited and Restricted Items policy may be offered the opportunity to participate in an informal resolution by completing this form.
The informal resolution process is optional. By completing this form, you are accepting responsibility for violating the Prohibited and Restricted Items policy and waive your right to a hearing. If you do not wish to accept responsibility or would prefer to meet with a staff member to discuss the incident and alleged policy violation, you must email communitystandards@govst.edu within 5 business days of receiving the policy violation letter.
To complete the informal resolution, you will need to review the Prohibited and Restricted Items policy and respond to the questions below. Please allow up to three business days for your submission to be reviewed. If your responses are not satisfactory, you may be asked to meet with a staff member to discuss the policy.
