This form should only be used to report unusual/bizarre/odd student behavior, student behavior that may violate HCC's Code of Student Conduct, or a potential Title IX  or 504/ADA concern.

The Retention Alert Form (available in Self-Service) is used to report concerns on behalf of students who may be struggling with personal challenges - in these cases, log into Self-Service to complete a Retention Alert. 

FEDERAL COMPLIANCE COORDINATION: HCC's Dean of Students serves as Title IX Coordinator for Students and 504/ADA Coordinator.
