Behavioral Incident (HEAT) Report

By completing this form, you are providing information to Harper College about a behavioral concern or incident.


The Harper Early Alert Team (HEAT) includes representatives from both divisions of Student Affairs: Student Engagement – Student Conduct and Title IX; Student Development - Advising, Counseling and Access and Disability Services; as well as the Harper College Police Department, and the Registrar's Office. The information from this form will be reviewed during regular College business hours and routed appropriately for follow-up. The information contained in this report may serve as the basis for a HEAT risk assessment, referral to campus partners, and/or the initiation of the student conduct process.
