Sexual or Gender-Based Misconduct (Title IX)


This form may be used to submit concerns about sexual or gender-based misconduct, including sexual harassment, sexual assault, stalking, and dating/domestic violence. The information you provide is sent directly to the Harper College Title IX Coordinator(s) for their review and response. The privacy of this information will be maintained to the greatest extent possible, while allowing the College to review and respond to the incident as needed to protect safety and promote accountability.

If you have experienced sexual violence and you are not sure whether you want to report to Harper College, you can call the 24-hour confidential hotline for Northwest CASA at 888-802-8890 and they can assist/support you during this process. You may use this form to report anonymously by not including your contact information.

You are encouraged to review Harper College's Title IX/Sexual Misconduct policy and resources so that you can learn more about your options for support/assistance, confidential and non-confidential reporting, and what the College's resolution processes involve. l If you have any questions, you can email the Title IX Coordinator(s). If this incident may also constitute a crime, you are encouraged to report it to the police, but you have the right not to do so. Learn more about your options if you experienced a crime.

Report to Harper Police

If you would like to initiate a criminal investigation, you are encouraged to file a formal police report by contacting the municipal police who have jurisdiction over the location where the incident occurred. If you are not sure where to go, you can call Harper Police at 847-925-6330 and they will assist you.

You can send a copy of this report to Harper Police which may help expedite the information sharing process, and provide Harper Police with background information if you feel unsafe on campus and would like assistance from campus police. Once Harper College Police receive this report, they may follow up with you and/or they may share it with other law enforcement agencies for investigation purposes.
