Dean Certifications/Background Checks


This form is for current and former students, or outside parties to request disciplinary records to be released to themselves or others. This includes transfer applications, background checks, and Dean's Certifications.

Information for Records Release

There are two types of Dean's Certifications. One is a letter from the Office of Community Standards, stating any conduct record you may have. The second is a form with a signed release form by the student that you attach at the bottom of the page. Dean's Certifications must be sent directly to the requesting agency.
