This form is used to report incident(s) of sexual harassment, sexual assault, sexual misconduct, interpersonal violence (including dating and domestic violence), stalking, sex discrimination (including discrimination on the basis of gender identity, gender expression, and sexual orientation), as well as discrimination based on race, color, age (40 years or older), religion, national origin, disability, Veteran status or retaliation for reporting a concern, filing a complaint, or cooperating in an investigation of a complaint.
Furthermore, Senate Bill 212 (SB 212), a Texas State Law that passed during the 2019 legislative session and became effective on January 1, 2020, requires all employees (both faculty and staff) at a public or private post-secondary institution to promptly report any knowledge of any incidents of sexual assault, sexual harassment, dating violence, or stalking "committed by or against a person who was a student enrolled at or an employee of the institution at the time of the incident". Individuals should submit this form to report a concern about themselves, or when they become aware, directly or indirectly, of an incident in which the victim or the offender is an HCC student, faculty, staff, contract employee, or visitor. Information provided may be from personal observation, a verbal or written report from a victim, or a verbal or written report through a third party who either observed or became aware of the incident.
Even if you are unsure if a specific behavior constitutes a violation, or that the alleged offender is an HCC student, faculty, or staff, you are encouraged to complete the form and provide all known information anyway.
It is unlawful to retaliate against a student, employee or any other person affiliated with the College for reporting a concern, filing a complaint, or cooperating in an investigation of a complaint.
A report can be made anonymously, without sharing your name. You may pick this option if you'd like to make the College District aware of incidents or general trends but you may not be seeking any further action. Depending on the information provided, the College District’s ability to respond to the report may be limited
