Thank you for taking the time to complete this Sycamores Support and Care Referral Form. Even if you are submitting multiple reports, please help us by taking the time to complete all relevant fields in each report, in both the Getting Started and Student Information Sections.
Please complete as much information as is known. This report will then be forwarded and reviewed by members of the Office of Student Support and Accountability by the next business day. If this is regarding Sexual Assault, Dating Violence. Domestic Violence, and/or Stalking you may complete this form, but it is NOT confidential. To speak to a confidential victim advocate, please call 812-243-7272. To report sexual misconduct, please complete the Title IX Report Form. If a student has a complaint/grievance with faculty, they may complete the Student Complaint/Grievance Form.
Financial Assistance Requests: Sycamore Support recognizes student may be referred to this office for financial assistance and will review any form submitted. Please note: Unless an unforeseen emergency (house fire, car stolen, tornado), this office does not have the funding to pay for items such as, but not limited to tuition, book fees, rent, utility bills, car payments, etc. For Financial Assistance, please contact the Office of Student Financial Aid for options and/or to review your financial aid options. This office will offer support to services and other sources if applicable.
Please be aware that a member of the Student Support and Accountability team will likely reach out to the student to offer support and assistance. Should you wish for this not to occur, please be very clear about this in your report. However, due to the significant health and safety risk either to the individual or to the university community, there may be an obligation to follow up with the student of concern, including revealing information shared in the referral. In addition, a staff member may follow up with you to possibly gain additional information or to inform you of next steps. Although details may not be able to be shared out of respect for the student's privacy, we wish to collaborate with you to nurture communication and ultimately intervention to support the student's academic and personal success.
