Indiana State University Policy 923 Non-Discrimination and Anti-Harassment prohibits discrimination, sexual harassment, sexual assault, dating violence, domestic violence, and stalking.
A Formal complaint is a document filed by the complainant (or by the Title IX Coordinator) alleging discrimination, sexual harassment, sexual assault, dating violence, domestic violence or stalking against an individual(s).
Filing a formal complaint will initiate the grievance process related to the reported allegations, which may include initiation of an investigation or mediation process.
Students identified in this Formal Complaint are afforded certain rights under the Family Educational Rights and Privacy Act (FERPA) which establishes requirements regarding the privacy of student records, including the right to inspect and review this document and any supporting documentation. Additionally, employees alleged to have committed a violation of university policy may also have similar rights to review this document and supporting documentation. All information you submit as part of this formal complaint becomes part of the record in this matter and is subject to review by the parties involved.
