When you submit this report, you are providing official notice to Kansas State University of an allegation of sexual discrimination or an allegation of discrimination based on a protected class/status.
When formally reported, allegations will be reviewed and addressed using the applicable policy and procedures. Allegations will be investigated in a prompt, thorough, and unbiased manner, and in compliance with applicable law. Following a formal report for allegations of sexual discrimination, information pertaining to the report will be disclosed to only those persons with a legitimate need to know, and to the extent authorized or required by law.
If you believe criminal conduct has occurred, you have the right to file a complaint with law enforcement. The criminal process and the policies utilized to investigate complaints submitted through this reporting form are separate procedures.
Additional information for completing this form:
1. Submission of this form does not automatically begin a formal investigation. A person submitting this form should expect to receive a response from OIE within 2 business days acknowledging receipt and next steps.
2. A formal complaint is not required to receive supportive measures or resources from OIE. Resources or supportive measures are both available to any party with or without the filing of a formal complaint on any type of report. 3. If this report contains information related to Title IX, including sexual harassment, sexual assault, stalking, domestic violence, or dating violence, the Title IX Coordinator will make contact with you within 2 business days via email regarding next steps and supportive measures that may be available to the involved parties.
