This form is to be used to submit a concern or complaint that students have been unable to satisfactorily resolve with the faculty, staff, students or others involved.

Complaints may be submitted anonymously; however, unless you include your contact information, Kirtland will be unable to investigate your complaint or respond back to you regarding the subject matter.
Keeping official tracking information on student complaints is required by the HLC Institutional Records of Student Complaints (FDCR.A.10.030)
